Head over to Adalo's website and log in. If you don't have an account yet, go ahead and create one. It's pretty straightforward.

On your dashboard, hit the "Create New App" button to kick off a new project. You can either pick a template or start from scratch. Totally up to you.

In your project, add a new screen by clicking the "+" button and selecting "Add Screen." You can go with a "Form" screen layout if it's there, or just a "Blank" screen if you want to customize everything.

Now, design your survey layout. Drag and drop components like text inputs, radio buttons, checkboxes, and sliders onto the screen. Arrange them in a way that makes sense for your survey.

Create a new collection (database) for your survey responses if you don't have one already. Connect each form component to this collection so user inputs are stored correctly. Click on a form component, select "Add Action," and then choose "Create" under the collection options.

Set up actions for when the form is submitted. These actions can include sending data to your collection, navigating to another screen (like a thank-you page), or triggering an email confirmation.

Customize the look of your survey using Adalo's styling options. Change colors, fonts, and sizes to match your desired look and feel.

Preview the app to test the survey. Make sure all inputs work correctly and that the data is stored in the right collection. Debug any issues that come up.

Once you're happy with the survey, publish the app by clicking on the "Share" button in the top right corner. Choose how you want to share it (e.g., web app, mobile app).

Share the app link to start collecting survey responses. Keep an eye on the data coming into the connected collection.
